چکیده

Purpose: Free-floating thrombus (FFT) of the aorta is very rare but has a high risk of distal embolization. While the necessity of treating such a condition is evident, the diagnostic and therapeutic modalities remain controversial. Thus, we reviewed seven cases of FFT of the aorta. Methods: A retrospective study was performed usings even patients diagnosed with FFT of the aorta at the Catholic University of Korea between January 1999 and December 2008. We excluded those patients who had thrombi with concomitant atherosclerotic or aneurysmal aorta. Results: The mean patient age was 59.6±13.6 years old. The male-to-female ratio was 3:4. Embolization to arteries of the extremities occurred in two patients and to visceral arteries in four patients. Of these seven patients, four were initially treated with anticoagulation, and two were initially treated with thrombectomy; one patient refused any kind of treatment. Of the four patients treated with anticoagulation, three experienced complete dissolution of the thrombi while anticoagulation proved ineffective in the remaining patient who subsequently underwent thrombectomy. In all of the three patients who had received thrombectomy, postoperative anticoagulation was employed. There was no recurrence of FFT of the aorta during the follow-up period. Conclusion: Were commend systemic anticoagulation with low molecular weight heparin (LMWH) as the first line of treatment for FFT of the aorta. If the thrombus persists or recurrent embolism occurs during anticoagulation therapy, surgery should be undertaken. (J Korean Surg Soc 2011;80:204-211)

Complicated course consequences of a floating thrombus in ascending aorta.

The ascending aorta may be the site of origin of systemic embolization of an unidentifiable cause. We report a case in which a free-floating thrombus of an unknown cause was detected in the ascending aorta via transesophageal echocardiography. The removal of this pedunculated thrombus, which was attached onto a macroscopically and histologically mildly atherosclerotic aortic wall, led to an une...

Floating Thrombus in the Ascending Aorta of the Patient with Systemic Sclerosis - A case report -

Aortic thrombi are important because it can cause the central and peripheral embolizations. Aortic thrombi can occur anywhere in the aorta but extremely rare in ascending aorta without atherosclerosis, aneurysm, cardiosurgical or traumatic state. Systemic sclerosis (SSc) is an autoimmune disorder of connective tissue and it can involve multisystem. Enhanced coagulation pathways, decreased fibri...

Fistula between the abdominal aorta and a retroaortic left renal vein: a rare complication of abdominal aortic aneurysm

http://dx.doi.org/10.1590/0100-3984.2016.0083 Because of the high risk of massive systemic embolization, it is considered necessary to treat a free-floating thrombus in the aortic arch. However, the ideal treatment remains undefined. Although the use of a thrombolytic is considered one of the options, it carries the risk of selective lysis of the pedicle of the lesion, which would have catastro...

Catching a floating thrombus; a case report on the treatment of a large thrombus in the ascending aorta

BACKGROUND The ascending aorta is an uncommon site for non-infective thrombus. In non-aneurysmal and non-atherosclerotic vessels this condition becomes extremely rare, while it represents a source of potential cerebral and peripheral embolic events. Currently, there is no consensus in the guidelines on how to treat a free floating thrombus in ascending aorta, therefore we present our decision m...

Unusual Case of a Free-floating Ball Thrombus with Preserved Attachment to the Left Atrial Appendage Causing Recurrent Obstruction of the Left Ventricular Outflow Tract

Few cases of a left atrial thrombus without mitral valve disease have been reported. We present an unusual case in which a patient presented to the emergency department with syncope and acute cerebral ischemia caused by a ball thrombus originating from the left atrium (LA). An emergency bedside echocardiogram showed the LA ball thrombus intermittently obstructing the mitral orifice and, at time...
